As treasurer of the South West Volleyball Association I have updated its expense claim form as the people were sending their claims to the previous treasurer (as the form had his address on it).
Rather than simply change the address I took the opportunity to simplify the form restricting which cells could be modified and in a lot of cases what values could be entered; for example only allowing a positive number for the number of miles.
I wanted to publish the form as an Excel template to encourage users to start a new form each time rather than copying a previous form. However the volleyball association uses Wordpress to manage its website and Wordpress does not allow an Excel template to be added to its media library. Therefore I am publishing the template here on my own website.
